In medicine there are increasing efforts to identify disease at an early stage and to intervene in order to prevent its progression. However, it is not always clear what criteria should be used to define “disease” and how to distinguish this from health. Furthermore, there is often uncertainty about what is the best approach for prevention or management of early disease. Drawing on personal experiences, as well as a research journey spanning population screening, behavioural and chronic disease epidemiology, this lecture attempts to unravel and discuss some of these concepts. I will specifically refer to research on cancer screening, childhood obesity prevention and COPD epidemiology.
